- Local community health centre (CLSC);
- Residential and long-term care centre (CHSLD);
- Rehabilitation centre (CR);
- Youth centre (CJ);
- Community organization;
- Ambulance company;
- Family-type resource;
- Intermediate resource;
- Health and social services agency;
- A private nursing home or private residence for the elderly.
Note: Should your complaint concern a professional that has neither a contractual nor an administrative link with an institution; it would not be examined under the Complaint Examination Process. However, it could be sent to his or her professional association.
